New Long-Term Condition (LTC) Recall System
What is changing?
- You will now be invited based on your birth month.
- Where possible, all of your long-term conditions will be reviewed together in one annual appointment, along with your medication review.
- This means fewer appointments and a more joined-up approach to your care.
Please note: Your review may be earlier or later than usual while we move to the new system.
Some patients may only be invited for blood tests and a blood pressure/BMI check before their results are reviewed by a clinician.
