Online Access To Medical Records
You can access your medical records via the NHS App.
The request will be reviewed by a GP and once authorized you will be able to access your coded data.
For Proxy access to someone else’s medical records if deemed appropriate i.e. your child, please fill in this online form
Subject Access Requests
If you would like to make a subject access request to obtain copies of your medical records kindly, please fill in this online form
Please be aware that repetitive requests will be charged.
